MH-E: support Mailutils "folders +/"

* lisp/mh-e/mh-utils.el (mh-sub-folders-parse):
Support Mailutils style of "folders +/" output.
* test/lisp/mh-e/mh-utils.el:
Test "folders +/" with GNU Mailutils 3.13.91 and later.
